There is no template for the written self-declaration. A letter on the letterhead of the organization declaring not being included in the UN Security Council 1267/1989 list, UN Procurement Division List or other UN Ineligibility List; will suffice - On the award is there possibility of splitting the order or you prefer one supplier to be awarded all line items?
Please note that some items are not laboratory related i.e. UPS.
Only one vendor will be awarded the contract - It is not clear from the RFQ documentation what delivery time is expected: 20 or 45 days?

Goods must be delivered in Juba by the 15th of December and paid for by the 31st of December - Item 5: Ethanol – what will be the application for Ethanol? Can the denaturated version be offered instead?
