Overview : ADDENDUM- NEW DEADLINE DATE IS 14th NOVEMBER, 2016 All companies that wish to participate in the process are required to complete the application forms indicating the categories for which the applications are being submitted available at the websites below. The entire information provided will be used as criteria to access the suitability of the various companies to qualify to receive either Request for Quotations (RFQ), Request for Proposal RFP and/or Invitation to Bid (ITB) to participate in the future procurement process for the services mentioned above. In addition to requirements in the pre-qualification application form, the interested applicants must provide the following documents: 1. Valid Registration certificates in Ghana 2. Valid Ghana Revenue Authority Tax Registration Certificate 3. Tax Clearance Certificate for the financial year ended 30th June 2016 4. Audited Accounts for at least the last three years indicating the annual turnover 5. CVs Management and technical Staff 6. Official Business physical, postal, telephone and email address 7. Proof of insurance of business and workman's compensation (where applicable) 8. Reference and recommendations from at least 3 present and 3 past client As part of the pre-qualification process, vendors are required to declare that they meet the pre-requisites for eligibility. Please note that both hard copies and/or soft copies of the applications will be accepted and it is the responsibility of the companies to ensure that the applica_ons are submitted successfully in an organized manner at bids.gh@undp.org for soft copies and hard copies must be deposited in a Tender Box located at the UNDP reception after completing and signing a register. The closing date for submittng a complete pre-qualification application form is 7th November, 2016. In case you submit an application form for pre-qualification but don't hear from us after 3 weeks of the closing date, it means your application has been unsuccessful and your application will not be advanced to the next stage of the procurement process.
