Overview :

The United Nations Development Programme (UNDP), invites Medical Institutions to submit a Proposal for the conduct of Medical Screening in Mabitac and Kalayaan, Laguna.

 SCOPE OF SERVICES:

The Medical Team will work together with the UNDP Regional Project team, under the supervision of the Regional Project Coordinator. The mission team leader shall consult and coordinate with the regional project team during the assignment. The mission team leader shall attend and participate in all meetings conducted by the regional project team related to the activity.

The Medical Team is expected to submit the following deliverables to the UNDP regional project team

  • Program Flow / Sessions
  • Health information, education and communication (IEC) materials (when necessary)
  • Conduct of health education
  • List of medicines to be procured
  • Assessment of the status of health and/or diagnosis of reported ailment(s) of the beneficiaries
  • Post evaluation of the medical mission activity (filled-in by participants) and summary report of the evaluation
  • Documentation of the medical mission activity conducted. This includes pictures and/or videos (when necessary)
  • Final written report (using the UNDP template)

DURATION / PERIOD OF WORK

The medical mission shall be implemented within 26 January to 09 February 2019.

QUALIFICATIONS

Institution

  • Must have SEC Registration/ Business Permit or similar documents;
  • Team of licensed general physicians, psychologists, therapists, and prosthetist/orthotist;
  • Institution should be in the business particularly in medical field for at least 10 years

Team Composition/Members:

  • Team Members must have at least 3 years of clinical practice:
  • Team should be composed of licensed general physicians, psychologists, therapists, and prosthetist/orthotist;
  • Team Members should be fluent in Filipino and English, both oral and written.

SCHEDULE OF PAYMENT

The Service Provider will follow the schedule of payments described below upon the accomplishment and submission of requirements.

  • all-inclusive fixed contract price supported by a breakdown of costs

Workplan and other related documents

  • Copy of legal documents such as SEC Registration, BIR Permit, Business Permit, or similar documents;
  • CV of Team Members, indicating all past experiences from similar projects;
  • Implementation Schedule (i.e. Gantt Chart or PERT/CPM)

UNDP reserves the right to reject any or all the proposals, waive any defect or informality therein, accept the proposal and award the contract to the most advantageous offer.  UNDP assumes no responsibility whatsoever to compensate or indemnify the proposer for any expense or loss that may be incurred for the preparation of proposals, nor does UNDP guarantee that an award will be made.
