Overview :

This Addendum is issued with the purpose to amend RFQ-382-20  as follows:

1- Reschedule the Mandatory Site Visit date for both location to Monday, 14-Dec-20 (10:00 AM – 12:00PM Iraq Time)

 

2- Bid submission deadline is extended from 13 Dec 2020, 14:00 hours to 17 Dec  2020, 14:00 hours (Iraq Time)

 

Companies should attend both site visits.

UNDP kindly requests your quotation for Rehabilitation of Qapusy PHC in Qapusy & Tel Qasab PHC in Tel Qasab Complex in Qapusy-Qayrawan-Sinjar-Ninawa Governorate as detailed in Annex 1 of this RFQ. 

This Request for Quotation comprises the following documents:

The bidder/s is requested to confirm in their bids that they have visited and examined the above sites and its surroundings, and obtained for themselves, on their own responsibility, all information which may be necessary for preparing the tender/bid and entering into a contract.

Please note that quotations must be submitted using Annex 2: Quotation Submission Form and Annex 3 Technical and Financial Offer, by the method and by the date and time indicated in Section 2. It is your responsibility to ensure that your quotation is submitted on or before the deadline. Quotations received after the submission deadline, for whatever reason, will not be considered for evaluation.
